General Purpose: The Community Health Registered Nurse (CHRN) supports the mission of the organization through the provision of high quality, cost-effective nursing care.


Essential Duties/Position Responsibilities:

  • Participates in the delivery of direct and indirect clinical care within the Registered Nurse scope and in accordance with organizational policies and procedures.
  • Nursing assignments may include: centralized medication refill processing, telephone triage, Chronic Care Management, Care Signal remote patient monitoring, Annual Wellness Visits, Care Team integration and support, among others. Assignments may evolve as organizational and patient needs change.
  • Collaborates with other departments, external organizations and consultants, including regulatory agencies and coalition partners, to support and coordinate patient care and to maintain STRIDE’s regulatory compliance.
  • Maintain patient confidentiality in accordance with Federal and State law and organization policies.
  • Document all patient care delivery in an accurate and timely manner.
  • Pursue educational opportunities to stay current in the practice of Ambulatory Nursing
  • Apply basic time management and patient care coordination skills to effectively provide nursing and health care services.
  • Other duties as required.

Education and Experience:

  • Required: Associate or Baccalaureate degree in Registered Nursing from an accredited school or training program.
  • Required: An active, unrestricted Registered Nurse license in the State of Colorado or appropriate compact licensing arrangement.
  • Required: 1 year of full-time experience as a Registered Nurse in a hospital or community health setting.
  • Previous home health or ambulatory care nursing experience is preferred.

Physical requirements include the ability to lift and carry up to 40 lbs, engage in activities like standing, walking, bending, kneeling, squatting, and climbing, as well as reaching overhead and repetitively. Sitting and extended periods of walking or standing are also required. The position involves mental demands such as comparing, copying, computing, compiling, analyzing, and coordinating tasks. Effective communication, instruction, interpersonal skills, and negotiation are crucial aspects of the role. Other physical and mental tasks may be required based on the nature of the work.

Work Schedule

We offer a competitive salary range of $72,000 - $96,000, depending on your experience.

This range reflects STRIDE’s good faith estimate of potential compensation at the time of posting. The final salary for the selected candidate will be determined based on several factors, including experience, education, budget, internal equity, specialty, and training.
