Position Summary:

The Wellness Coach works independently in the field with people of diverse ethnicities, backgrounds and preferences who have a mental illness and are living in the community, primarily, in single and scatter-site apartment and SRO housing. The Wellness Coach will develop and monitor individual service plans in conjunction with agency consumers, identify and document any issues in daily living skills. Play a significant role as a member of the social service team in the development of off-site recreation, socialization, educational and skill building activities with a view towards community inclusion and integration. Participation in ongoing individual and group supervision, team meetings, case conferences, division meetings and trainings is required.

Qualifications
  • Bachelor's Degree in human services or related field required 
  • A minimum of two years' experience working with people who have a mental illness
  • Computer literate in word processing and excellent writing capabilities to develop treatment plans
