Job description

Overview

In collaboration with patients, families (as defined by the patient), and staff across all disciplines and departments will work with the Clinical Nutrition Manager to plan, implement, and evaluate diabetes related nutrition services and programs. This position will focus on providing Diabetes Self-Management Training (DSMT) for patients who have confirmed Type 1 or Type 2 diabetes before pregnancy or who are newly diagnosed with Gestational Diabetes Mellitus, and will assist with other diabetes related initiatives as needed.

Responsibilities

Essential Duties (at least 5 that are non-negotiable duties and are absolutely pertinent to successfully completing the job without accommodations): Educate patients on food selection to maintain and promote health as well as to prevent and manage diabetes and associated comorbidities Train patients with diabetes on glucose monitoring and prescribed medication use May provide information and education to staff members including nutrition education Evaluate the dietary and lifestyle needs of the patient, collaborate with the patient to develop and implement a plan, and document all pertinent information into the patient's electronic medical record. Provide information and education to staff members using nutrition education materials or in-service meetings per request May implement and monitor quality control programs May participate in or manage special projects/programs, as needed Proficient in speaking, reading, and writing in Spanish (preferred) Perform other duties as assigned

Knowledge/Skills/Abilities: High level of skill at building relationships and providing excellent customer service. Basic computer systems knowledge, including but not limited to Microsoft Office products and the use of electronic medical records. Strong attention to detail and accuracy. Excellent verbal and written communication skills. Ability to apply interpretations of federal, state and local regulations. Ability to examine and evaluate the need for DSME services and programs. Ability to drive a vehicle and travel to various clinic locations, as needed

Qualifications

Education:

  • Bachelor's Degree in Nutrition or Dietetics (Required)

Experience:

  • 2 years of professional experience as a Registered Dietitian (Required)
  • Work in an outpatient or community health setting, Experience providing Diabetes Self-Management Training (DSMT) and medical nutrition therapy (MNT) in an inpatient/outpatient, individual and group education setting, bilingual English/Spanish (Preferred)

Licenses and Certification:

  • Registered Dietitian (RD) credential from the Commission on Dietetic Registration [CDR] (Required Upon Hire)
  • Current Certified Diabetes Care & Education Specialist [CDCES] from the Certification Board for Diabetes Care & Education Specialists [CBDCES] (Required Upon Hire)
  • Current license in the state of Texas as a Licensed Dietitian [LD] (Required Upon Hire)
  • Current Health Care Provider Cardiopulmonary Resuscitation [CPR] certification through American Heart Association or American Red Cross. Will be required to maintain a current CPR certification during employment. (Required Upon Hire)
