    Job description

    Responsibilities:
    • Caseload and/or billing expectations and requirements must be met on a monthly, quarterly, and yearly basis.
    • Maintains a caseload established with the Clinical Administrator in order to work closely with SDMI programs in assigned service areas. The actual caseload composition of any given Adult Case Manager will be determined by the intensity of his/her caseload, the geographical area, and the specific service structure in the region.
    • Assesses and evaluates adults to determine the appropriateness and need for case management and SDMI Waiver services.
    • Works within the Case Management Team and in collaboration with SDMI Waiver Clinical Lead.
    • Provides assistance in accessing and obtaining needed services and benefits.
    • Assists in the person's entry into service and monitors the needed services throughout service system.
    • Provides coordination and comprehensive development of an Individual Treatment Plan (ITP) and monitors that plan as required by established rules.
    • Follows State of Montana and AWARE rules, policies, procedures, and guidelines as they apply to this service.
    • Prepares reports, applications, and supporting documentation as requested by the supervisor within established time lines.
    • Perform other duties as assigned; attends and participates in mandatory training, in-services, staff meetings, and Corporate Committees as assigned. Upon hiring, the CFS will undergo training and will be expected to pass testing in the following areas: H.E.L.P (de-escalation and therapeutic holds), First Aid/CPR, Blood Borne Pathogens, Safety Health Emergency and Disaster (SHED) Safety, Safety Data Sheet (SDS), HIPPA, Professional Care Giver, and AWARE's Behavioral Competency (ABC);
    • Must demonstrate - on a consistent basis - the willingness and ability to productively utilize work time in order to meet the needs of the individuals being served as directed, with the understanding that specific fiscal goals (billing, case load number maintenance, weekly) must be met on a monthly basis;
    • Records notes, client contacts, assessments, referrals, and other data entry assignments via AWARE's Electronic Healthcare Record System.
    • Other Duties as assigned.
    • Some coverage may be out of the Bozeman area, including Billings.

    Requirements
    Talents, skills, and abilities:
    • Ability to, under AWARE's Unconditional Care Philosophy, develop and implement individual treatment plans with the knowledge of behavioral, clinical, and mental health interventions, skill building techniques, and understanding of the emotional and behavioral needs of assigned clients.
    • AWARE's SDMI Waiver Adult Case Manager must have the interpersonal oral and written communications skills to work as a team member, to make observations and to report them accurately, to learn behavioral, clinical, and mental health care methods, and to follow oral and written instructions.

    Minimum Qualifications:
    • A BS/BA Degree in a social work-related field and minimum of one (1) year of direct work experience in adult mental health case management.
    • One (1) year experience in the human service field.
