General

  • .4 FTE role
  • Fully inpatient consult service covering all three hospital campuses 
  • Average of 12 patients per day; no nights or weekends
  • Perform geriatric consultations received from providers at UPH-DM
  • Generate recommendations based on assessment of geriatric patient’s needs, with the goal of improved overall care, prevention of delirium, and decrease in functional decline
  • Document consultations, recommendations, plans, and follow up in the EHR
  • Provide education to providers and nursing on evidence-based care of geriatric patients in the acute setting
  • Serve as a geriatric liaison to the Trauma department, assisting with development and implementation of geriatric protocols and patient consultation
  • Partner with other disciplines to improve the care of geriatric patients at UPH-DM through protocol development, case review, consultation, etc
  • Track and monitor data that demonstrate progress toward goals determined for the support of geriatric patients at UPH-DM. These include LOS, falls, sitter use, M&M, delirium rate, staff & patient/family satisfaction
  • Partner with Hospital Elder Life Program (HELP) for additional support of Geriatric patients
  • Provides specialty specific patient-family teaching and counseling based on needs, and in collaboration with the care team

  • AGACNP (Adult Gerontological Acute Care Nurse Practitioner) or AGPCN (Adult Gerontological Primary Care Nurse)
  • Board Certified
  • 3 years of experience in an acute setting as a nurse or NP required
  • Experience working with elderly individuals required
  • Hold an unrestricted medical license in the state of State
  • Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
  • Eligible to work in the United States
