Position Overview

Sevita Health System is actively seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our compassionate team. Work Environment

The successful candidate will work remotely, primarily serving patients at our Detox Center located in Aurora, Illinois. This role requires flexible scheduling, with a rotating timetable that may include evenings and weekends to accommodate client needs. As part of our virtual team, you will collaborate regularly with colleagues, access digital resources, and engage in remote meetings to ensure seamless care delivery.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to evaluate patients' mental health, personal strengths, and treatment needs, ensuring a tailored approach to care.
  • Provide evidence-based therapeutic modalities including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) to address a diverse range of mental health disorders.
  • Collaborate closely with psychiatrists, nurses, and other clinical professionals to create a coordinated care plan that aligns with our patients' treatment goals.
  • Educate patients and their families about mental health diagnoses and treatment options, promoting understanding of medications and ongoing care strategies.
  • Facilitate individual and group therapy sessions, utilizing evidence-based practices to support coping mechanisms and emotional resilience.
  • Develop and implement personalized treatment plans in collaboration with the treatment team to reflect patient aspirations and choices.
  • Aid patients in navigating the complexities of the mental health system, advocating for their rights, and connecting them to necessary resources.
  • Facilitate therapeutic groups and educational sessions that empower patients with knowledge and tools necessary for their path to recovery.
  • Engage in discharge planning meetings to ensure a smooth transition out of care and continued support as patients reintegrate into their communities.

Qualifications
  • A Master's degree in Social Work from an accredited institution is required for this position.

Preferred Experience
