About the Role
Pathways is seeking a compassionate, organized, and detail-oriented Care Coordinator to support individuals receiving services through our Certified Community Behavioral Health Clinic (CCBHC). The Care Coordinator plays a vital role in ensuring consumers receive seamless, integrated care by connecting them with behavioral health, physical health, and community-based resources that promote wellness, recovery, and overall quality of life.
This position works collaboratively with consumers, providers, and community partners to coordinate services and support a person-centered plan of care.
Education Requirements
- Minimum Associate Degree or Diploma in Nursing required
- Current Kentucky nursing license required
Experience Requirements - Experience working with and accessing community resources
- Proficiency in accurate and timely documentation within an electronic health record (EHR)
- Demonstrated ability to collaborate effectively with multidisciplinary teams
- Experience working with special populations served under CCBHC guidelines preferred
- Strong organizational skills and ability to manage multiple priorities simultaneously
- Experience in public health, care coordination, social services, or behavioral health settings preferred
Essential Duties and Responsibilities - Coordinate services for consumers across physical health, behavioral health, and community support systems
- Facilitate linkages to community resources, including:
- Social services
- Housing assistance
- Educational programs
- Employment services
- Healthcare providers
- Other support services as needed
- Ensure smooth transitions of care between providers and levels of service
- Develop and maintain collaborative relationships with community partners and service providers
- Support consumers in achieving goals identified in their person-centered plan of care
- Document all care coordination activities accurately and timely within the electronic health record
- Monitor consumer progress and assist in addressing barriers to treatment and recovery
- Participate in multidisciplinary treatment team meetings and case consultations
- Maintain compliance with CCBHC standards, agency policies, and regulatory requirements
- Advocate for consumer needs while promoting independence, wellness, and recovery
