    Care Coordinator

    Salary: $44,000 annually

    About the Role

    Pathways is seeking a compassionate, organized, and detail-oriented Care Coordinator to support individuals receiving services through our Certified Community Behavioral Health Clinic (CCBHC). The Care Coordinator plays a vital role in ensuring consumers receive seamless, integrated care by connecting them with behavioral health, physical health, and community-based resources that promote wellness, recovery, and overall quality of life.

    This position works collaboratively with consumers, providers, and community partners to coordinate services and support a person-centered plan of care.

    Education Requirements
    • Minimum Associate Degree or Diploma in Nursing required
    • Current Kentucky nursing license required
    Experience Requirements
    • Experience working with and accessing community resources
    • Proficiency in accurate and timely documentation within an electronic health record (EHR)
    • Demonstrated ability to collaborate effectively with multidisciplinary teams
    • Experience working with special populations served under CCBHC guidelines preferred
    • Strong organizational skills and ability to manage multiple priorities simultaneously
    • Experience in public health, care coordination, social services, or behavioral health settings preferred
    Essential Duties and Responsibilities
    • Coordinate services for consumers across physical health, behavioral health, and community support systems
    • Facilitate linkages to community resources, including:
      • Social services
      • Housing assistance
      • Educational programs
      • Employment services
      • Healthcare providers
      • Other support services as needed
    • Ensure smooth transitions of care between providers and levels of service
    • Develop and maintain collaborative relationships with community partners and service providers
    • Support consumers in achieving goals identified in their person-centered plan of care
    • Document all care coordination activities accurately and timely within the electronic health record
    • Monitor consumer progress and assist in addressing barriers to treatment and recovery
    • Participate in multidisciplinary treatment team meetings and case consultations
    • Maintain compliance with CCBHC standards, agency policies, and regulatory requirements
    • Advocate for consumer needs while promoting independence, wellness, and recovery
    Knowledge, Skills, and Abilities
    • Strong interpersonal and communication skills
    • Ability to build rapport with consumers from diverse backgrounds
    • Knowledge of community resources and referral processes
    • Excellent organizational and time management skills
    • Ability to maintain confidentiality and professional boundaries
    • Proficiency with electronic health records and Microsoft Office applications
    • Commitment to person-centered, trauma-informed, and recovery-oriented care
