Navajo families farm centuries-old field in Canyon de Chelly National Monument, site of prehistoric cliff dwelling inhabited by the Anasazi, ancestors of today's Pueblo Tribes. The Facility - Shiprock Service Unit
The Shiprock Service Unit in Shiprock, New Mexico includes the Northern Navajo Medical Center (NNMC), Four Comers Regional Health Center (FCRHC), Dzilth-Na-O-Dith-Hle Health Center (DZHC), Sanostee and Toadlena Health Centers and is located in the Four Corners area of the United States where New Mexico, Arizona, Colorado and Utah meet. Northern Navajo Medical Center is one of five facilities located within the Shiprock Service Unit. The Shiprock Service Unit is the largest service unit of the Navajo Nation.
Approximately 80,837 Native Americans (mostly Navajo) are enrolled at Northern Navajo Medical Center. The inpatient load is 29 per day and the outpatient volume averages 600 per day.
A variety of services are available including internal medicine, family practice, urgent and emergency care, pediatrics, general surgery, women's health, mental health, respiratory therapy, outpatient rehabilitation services, pharmacy, optometry, dental, public health nursing, social services, health promotion/disease prevention, radiology, laboratory and benefits coordination.
Northern Navajo Medical Center employs 860 full-time employees. The medical staff consists of 75 physicians and 24 mid-level providers.
General requirements (Duties and Qualifications include but are not limited to):
  • Establish, direct, coordinate and evaluate the Service Unit's Risk Management/Patient Advocate Program.
  • Develop, plan, and administer an internal risk management plan.
  • Ensure the collection and analysis of data to monitor the performance of processes that involve risk or that may result in serious adverse events.
  • Analyze data collected on adverse events, near misses, and potentially unsafe conditions.
  • Ensure compliance with data collection and reporting requirements of governmental, regulatory, and accrediting agencies.
  • Facilitate and ensure the implementation of patient safety initiatives such as improved tracking systems for preventive screenings and diagnostic tests, medication safety systems, and falls prevention programs.
  • Systematically monitor and evaluate the quality of patient care, continuous improvement of patient care and support of service unit strategic plan.
  • Conduct health system risk/quality management surveys and analysis.
  • Monitor the effectiveness and performance of risk management and patient safety actions. Performance monitoring data may include:
    • Claims and claim trends Culture of safety surveys Event trending data
    • Ongoing risk assessment information
    • Patient's and/or family's perceptions of how well the organization meet s their needs and expectations Quality performance data
    • Research data
  • Determine risk events from collected information and provide specific recommendations to the Executive Leadership Team for evaluation and action.
  • Provide early analysis and investigation of actual as well as potential risks within and around the area served.
  • Communicate with the Chief Executive Officer (CEO), Chief Medical Officer, Chief Operating Officer, Chief Financial Officer, Chief Nurse Executive, and other key staff members as needed. Inform and provide input to all regulatory issues/findings/surveys to the executive team department managers to eliminate all levels of risks; assist clinical directors and department managers in designing safe, quality care processes.
  • Discuss and make decisions on policy making involving the hospital self-assessment programs concerning potential risk.
  • Respond to crisis situations that have risk management implications to assist staff. Answer medical/legal inquires of physicians, nurses, and administrators regarding emergent patient care issues and loss control.
  • Interact with legal counsel, patients/families.
  • Work in close alliance with attorneys who present subpoenas. Provide requested records for review and works with the US attorneys who represent the facility in regard to Tort Claims. Assist in preparation of staff involved in the Tort Claim process who may be called for deposition or trial.
  • Chair the Risk Management Committee. Serve as member of the Quality Management Team, Corporate Compliance Committee, Safety Committee, and coordinate regularly with Health Information Systems reviewing subpoenaed records for completeness and provides analysis to NAIHS for approval. Incumbent coordinates with the compliance officer concerning regulatory requirements and protection of patient rights, HIPAA and informed consent.
  • Conduct or participate with other department heads in the orientation of employees concerning the Risk Management program utilized by the hospital. Develop formal program for ongoing education for hospital staff.
  • Serve as Community Liaison.
  • Represent Service Unit at any NAIHS or Client meetings concerning Risk Management issues.
  • Provide oversight, guidance and directly supervises (1) Patient Advocate (Client-08) and indirectly supervises subordinate staff, including temporaries, students, volunteers, student trainee.
  • Plan and assign work schedules and priorities to meet the program requirements.
  • Independently plan and organize staff work or priority projects/assignments and provides general advice on all unique situations or requirements. Monitor, evaluate, and correct employee performance. Establish, write, and develop an employee evaluation process and takes corrective action when required to assure safe quality patient care.
  • Provide supervision to employee, including counseling, disciplinary action, corrective action, evaluation, and overall management of the employee. Resolve informal complaints of employee and refers those beyond to higher level of management if needed.
  • Plan, organize, implement, and follow through on quality improvement/risk management/patient safety team projects.
  • Maintain accurate employee files, which will include licensure, mandatory staff development presentations, training certificates, competency, orientation, etc. as dictated by the SRSU Policy and Procedure.
  • Prepare various program summary reports/correspondence to communicate program matters, issues, and concerns to appropriate Client Agency Officials, Management and facility staff.
Qualifications
  • Bachelor's degree in nursing (required).
  • Masters in Nursing (preferred).
  • Minimum 2 years experience as a Risk Manager in a hospital setting including supervisory experience.
  • Professional knowledge of the Indian Health Services as a whole.
