Job Summary We are hiring an experienced Palliative Care Manager (LMSW) to lead the development of a new palliative care program in Grand Rapids, MI. This is a full-time leadership opportunity for a licensed social worker with expertise in hospice or palliative care, program development, and team management. The manager will oversee program design, staff coordination, and patient care initiatives to ensure compassionate and high-quality service delivery. Key Responsibilities Lead program development for a new palliative care service in Grand Rapids Oversee patient care initiatives ensuring quality and compassionate delivery of services Collaborate with interdisciplinary teams including physicians, nurses, and social workers Manage staff through leadership, mentoring, and training initiatives Ensure compliance with regulatory standards and hospital policies Utilize EMR systems and Microsoft Office tools for documentat ion and reporting Required Qualifications Master’s degree in Social Work (MSW/LMSW) Minimum 3 years of relevant experience in hospice or palliative care Proficiency in EMR systems and Microsoft Office Strong communication skills and team leadership abilities Preferred Qualifications Leadership experience in program development or management Background in team coordination within hospice or palliative care settings Certifications Active LMSW license required Education: Masters Degree
