Job Overview
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care in a Long Term Care and Skilled Nursing environment. The LPN will collaborate with residents, families, and interdisciplinary healthcare teams to create a safe, comfortable, and therapeutic environment while delivering direct and indirect patient care.
Unit Overview
The position may provide care across multiple units, including:
Memory Care Unit serving residents with Alzheimer's disease and dementia in a supportive, home-like environment.
Transitional Rehabilitation Unit providing short-term skilled nursing and rehabilitation services for patients recovering from orthopedic procedures, stroke, surgery, and other acute illnesses or injuries.
Long-Term Care Unit supporting residents requiring ongoing nursing care and assistance with daily living activities.
Key Responsibilities
Provide direct nursing care and administer medications in accordance with physician orders and facility policies.
Monitor, assess, and document resident conditions, reporting changes to the healthcare team.
Assist with treatment plans, wound care, IV therapy support, and rehabilitation-focused nursing care.
Maintain accurate patient records and documentation within the electronic medical record system.
Collaborate with nurses, physicians, therapists, and other care providers to ensure quality patient outcomes.
Promote resident safety, comfort, dignity, and independence.
Adhere to infection control, safety, and regulatory standards.
Float to other units as needed to support patient care operations.
Requirements
Graduate of an approved Licensed Practical Nursing (LPN) program.
Current active State or Compact LPN License.
BLS certification required.
Previous Long Term Care, Skilled Nursing, Rehabilitation, or Geriatric Nursing experience preferred.
Knowledge of medical terminology and nursing best practices.
Strong communication, teamwork, and organizational skills.
Ability to adapt to changing patient needs and work effectively in a fast-paced environment.
