Join our dynamic and energetic team of professionals committed to providing high quality Case Management services to adults (21+) with developmental and/or intellectual disabilities and/or Traumatic Brain Injuries (TBI).

Community Partners is looking for energetic, upbeat and motivated candidates dedicated to helping others and who enjoy working as part of a collaborative and supportive team to fill the role of Case Manager for Developmental Services.

As a Case Manager for Developmental Services, you will have the opportunity to help individuals with developmental/intellectual disabilities and/or traumatic brain injury to be fully engaged in their communities, to develop the skills to be as independent and productive as possible and to have enriching and high quality life experiences.

The ideal candidates for this position will have:

  • Associate's Degree required. Bachelor's preferred.
  • 2-3 years of relevant field experience preferred.
  • Ability to communicate effectively both orally and in writing
  • Use of provided technology to perform some functions of the position.

This is a 40 hour per week position with, flexible schedule and an excellent benefits package that includes medical, dental, paid-time off, flexible spending accounts and more!

$1,000.00 Sign On Bonus!!The Role:
As the Case Manager, you will be responsible for organizing, facilitating, and documenting the service planning for individuals with Developmental Disabilities as well as working collaboratively with other community resources to ensure the quality of service provision. 


What we're looking for: 
Associate degree or equivalent required, Bachelor's Degree preferred
Two-three years of relevant field experience preferred
Excellent oral and written communication skills 
Ability to utilize technology provided to perform requirements of the job
Hold a valid driver's license and proof of vehicle liability coverage.
Must demonstrate excellent multitasking abilities.
