Available Shifts:  Day, evening, or night shift with variability as needed by program    

SUMMARY:  The On Call Recovery Mentor provides and coordinates recovery support services in the community for participants.  This position works directly with participants and on behalf of the individual to reinforce treatment goals and access resources in the community, promoting a recovery-oriented lifestyle. This will be an on-call position to cover call-outs and vacation time in sobering.

ESSENTIAL RESPONSIBILITIES AND TASKS: 

• Assist participants in accessing shelters, medical and/or emergency services and other community resources, such as sourcing food, clothing, substance use treatment and withdrawal management, mental health services, utilities assistance.
  • Asist participant with resources and/or referrals to community resources base on needs.
• Complete needs assessment and SUD triage tool
• Assist individual with meeting sobering goals.
• Consistent attendance and punctuality are required of this position.
• Facilitate connection to the recovery community and other supports that may reinforce the goals of sobering.
• Teach and model positive recovery behaviors while maintaining appropriate professional boundaries.
• Facilitate peer recovery and life skills groups if needed in withdrawal management.
• Build and maintain positive working relationships with individuals and organizations in the recovery community.
• Participate in team meetings, case conferences, work groups, committees, trainings, and applicable community organized events. 
• Use agency network and electronic health records to document all relevant contacts with or in regard to patients.
• Promote community integration and positive outcomes in the areas housing, employment, education, family and social support and recovery.
• Assist in the tracking of critical data elements necessary for program reporting.
• Accompany and may drive patients to recovery related appointments.

SECONDARY RESPONSIBILITIES AND TASKS: 

• Assist the team by providing short-term relief coverage and/or basic training for other staff members.

• Ability to be attentive to and identify high risk situations, communicating with the appropriate channels in a timely manner.

• Ability to demonstrate a non-judgmental and respectful attitude with patients at all times.
• Ability to establish rapport quickly, maintain appropriate boundaries, and remain nonjudgmental to patients.
• Ability to utilize both oral and written communications skills to effectively communicate with patients, co-workers, community resources, supervisors, and various other individuals encountered in the course of performing work while maintaining patient confidentiality and establishing professional rapport. 
• Ability to maintain professional communications and boundaries while managing complex situations and interacting with individuals diagnosed with substance use or co-occurring conditions.
• Ability to operate a computer, utilize basic MS Office programs, and effectively navigate and maintain patient records in an electronic medical records system.

REQUIRED EDUCATION AND EXPERIENCE:

• High school diploma or GED.

REQUIRED CERTIFICATIONS AND LICENSES:

• CRM certification at time of hire 
• May be required to attain a Basic Life Support (BLS) certification or ability to acquire within 90 days of hire.
• Crisis Prevention training completion within 90 days of hire.
• A valid Oregon or Washington driver’s license

PHYSICAL DEMANDS: 

• Must be able to safely operate a motor vehicle, including a passenger van for residential settings.
• Must be able to perform Basic Life Support for 15 minutes continuously if certified in BLS.
• Must be able to lift up to 25 lbs.
• Must be able to follow infection control practices and safely work in patient-focused environment.
• Must be able to negotiate a staircase, reach, and handle objects.
• Must be able to sit for extended periods of time
• Must be able to operate a personal computer for extended periods of time

OTHER REQUIREMENTS: 

• Ability to work independently and within a multi-disciplinary team.
• A negative result on all pre-employment, reasonable basis, and post-accident drug screens.
• An acceptable criminal record as determined by DHS, the Oregon Administrative Rules, and the Oregon Revised Statutes; and no current supervised parole or probation.
• An acceptable driving record and automobile insurance coverage as recommended by CODA’s automobile insurance provider (Withdrawal Management Program may or may not require).
