We provide primary care and other services to seniors and families in our medical centers that are comfortable, convenient and staffed with caring professionals. Qualifications:
  • Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
  • Must have an active and unrestrictive TexasAPRN license
  • Must have DEA license or eligible to obtain it within 2 months of date of hire
  • Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
  • Excellent documentation and patient relationship-building skills
  • Proficient with EMR systems (eCW a plus!)
