Your Opportunity:

Assisted Living Alberta at the Sturgeon Community Hospital is looking for a Social Worker to support patients and families in inpatient/emergency department settings and be an integral part of department changes. This position requires a high degree of critical thinking, strong knowledge of Social Work scope and role, and a passion for Social Work excellence. You will be required to prioritize workload, formulate assessments and create intervention plans in a short timeframe. These plans include psychosocial and mental health interventions, risk management, and resourcing to support ongoing treatment and/or discharge planning. The Social Worker will demonstrate exceptional communication skills and ability to work collaboratively with caregivers, families, and community organizations. This position works collaboratively with multidisciplinary teams and requires competence in approaching questions about decision-making and capacity. In addition, working knowledge of discharge planning, community resources, financial supports and processes related to transitions to alternate levels of care are an asset.

Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Previous experience working in mental health, or with mental health assessments. Ability to problem-solve and make time-sensitive decisions using critical thinking. Must be able to tolerate long periods of sitting, standing, and repetitive movements.
