Hospice RN Case Manager (PRN)

Greater Houston 

The Job:

A hospice RN case manager plays a crucial role in providing compassionate care to patients with terminal illnesses, coordinating their treatment plans, and supporting their families throughout the end-of-life journey.

Responsibilities:

  • Patient Assessment; Care Coordination; Direct Nursing Case; Education and Support; Documentation and Compliance; Advocacy

Must Haves:

  • RN Registered Nurse in Texas or Compact state in good standing on license. A valid RN license is mandatory, along with certifications in hospice and palliative care being advantageous.
  • Experience with RN Case Management in Hospice of at least one (1) year.  Prior experience in nursing, particularly in hospice or palliative care settings is needed.
  • Skills in crisis management and interdisciplinary teamwork are essential.
  • Typically, a hospice RN case manager holds a Bachelor’s or Master’s Degree in Nursing, with relevant coursework in palliative care and healthcare management. Not required but a plus!
  • Must have experience in HCHB EMR

Work Environment

  • Hospice RN case managers often work in home health settings, providing care in patients' homes, which allows for a more personalized approach to end-of-life care.
