Work Environment

This position is onsite at our assisted living facility in Binghamton, New York, where you will work closely with a multidisciplinary team of healthcare professionals. Key Responsibilities
  • Conduct detailed and comprehensive health assessments to create tailored care plans for each resident.
  • Diagnose and manage both acute and chronic diseases, implementing preventive care strategies and health promotion activities.
  • Collaborate effectively with other healthcare providers, caregivers, and community stakeholders to ensure optimal care delivery.
  • Advocate for residents and their families, ensuring access to necessary resources and healthcare services.
  • Coordinate referrals and follow-up appointments, maintaining continuity of care as residents transition between services.
  • Document all patient information accurately using electronic health record (EHR) systems, ensuring compliance with legal and regulatory standards.
  • Participate in community outreach events and health fairs aimed at raising awareness of health issues and providing screening services.
  • Monitor the evolving landscape of community health trends and best practices, integrating relevant knowledge into your care approach.
Patient Population / Client Focus

As a Nurse Practitioner at our assisted living facility, you will predominantly serve a diverse population consisting mainly of elderly residents. This population often presents with complex healthcare needs, including chronic illnesses prevalent among seniors. Your role will be instrumental in promoting healthy aging while guiding individuals and families through the nuances of age-related health concerns.

Qualifications
  • A Master’s or Doctoral degree in Nursing from an accredited institution.
  • Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
  • A valid, unrestricted Nurse Practitioner license in New York State, with a current DEA and NPI number.
  • Demonstrated ability to utilize EHR systems proficiently.
  • Exceptional clinical assessment, diagnostic, and critical thinking abilities.
Preferred Experience
  • Minimum of two years of experience in a primary care or family medicine setting; experience in community health or public health is highly desirable.
  • Strong collaboration and communication skills, allowing for effective interaction with a team and diverse populations.
  • Proven ability to work independently, demonstrating self-motivation and accountability in patient care.
