  • ย Completes initial comprehensive and ongoing assessment of patient and family to determine best services to meet their needs
  • Coordination of services with the patient, family, and care giver team.
  • Ongoing supervision of all professional and para-professional staff assigned to the patient.
  • Ongoing communication with patient and family to ensure client satisfaction and best outcomes

Requirements

  • Registered Nurse (RN) license NYS
  • BSN required
  • 3 + years of clinical experience
  • Home care experience
  • Minimum 1 year of supervisory experience
  • Excellent observation, problem solving and communication skills
