Job Summary:

Carease Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Brackenridge, Pennsylvania. As a Nurse Practitioner, you will deliver high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home.

Responsibilities:

• Provide preventive care to residents in skilled nursing facilities, with a focus on Geriatric care and Internal Medicine
• Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation
• Build strong relationships with facility staff and maintain a patient-centered approach
• Collaborate with healthcare teams to develop and implement personalized care plans
• Conduct thorough assessments, diagnose and treat patients, and develop treatment plans
• Maintain accurate and timely records, adhering to facility and regulatory requirements
• Participate in education and training programs to stay current with best practices and emerging trends

Requirements:
