Job description

Scope:

The Transitional Assistant provides supervision, education, training and various support services to the residents of Skyland Trail's Transitional Program.

Duties:

  • Ensures that the clients maintain their rooms in a safe, neat, and sanitary manner by completing room/apartment checks bi-weekly; provides training/instructions; assists clients with room cleanings as needed and identify areas to be included in wellness/recovery plans.

  • May complete Activities of Daily Living assessments: identify strengths and weaknesses to be included in wellness/recovery plans.

  • Instructs clients in activities of daily living, interpersonal, and socialization skills.

  • May transport clients when needed to outings or treatment.

  • May conduct assigned Transitional Housing groups and document based on written curriculum for each group.

  • Co-facilitate community meetings with other transitional staff.

  • During evening shift hours, assist clients with preparation of dinner and clean up of the kitchen and dining area.

  • Facilitate evening client activities.

  • Utilize staff shift log to convey pertinent information to other staff.

  • Document pertinent information re: recovery goals on shift change report and email to clinical team.

  • Monitors clients for any unsafe behavior or violations of rules, intervenes when necessary and communicates any concerns to the team.

  • Ensures that the clients are following safety procedures at all times in the residence and while riding in the van, (ie. seat belts on, no drinking, no smoking, no rough-housing)

  • Ensures that the facility is maintained according to standards as dictated by Skyland Trail Policies and accrediting bodies at all times, (Joint Commission).

  • Ensures that the facility maintains food items, supplies, and other items as necessary for the continuity of the program, (makes purchases as approved by the Director and/or Program Manager.

  • Responsible for driving in a safe and responsible manner at all times, obeying all traffic laws.

  • Completes room searches as needed.

  • Completes apartment inventory forms when clients enter/leave the program.

  • Responsible for completing required vehicle inspection checks as outlined in transportation policies.

  • Assists maintenance/housekeeping staff in cleaning and resetting rooms/apartments as needed.

  • Supports residential staff if urgent issues arise at either of the residential facilities.

  • Assists/completes urine drug screens or alcohol tests for clients as needed.

  • Responsible for monitoring and reporting maintenance needs of the buildings.

  • Assists in crisis situations in coordination with the crisis communication plan, (on-call counselor)

  • Provides good customer service at all times.

  • Conducts emergency preparedness procedures per policies.

  • Completes incident report per policies.

This position should have sufficient preparedness in the following areas:

  • Good driving record
  • Current CPR & First Aid certifications, obtainable upon hire if not already held
  • Knowledge of self-administration of medication procedures
  • Crisis Management skills
  • Group development/facilitation skills
  • Good customer services skills
  • Knowledge of emergency procedures for fire, disaster and unplanned incidents
  • Documentation skills

Qualifications:

  • Minimum of a Bachelors degree in related field, (i.e. Psychology, Social Work, Counseling, etc) and/or (2) years of clinical Experience in the mental health field.
  • Prior experience with the chronically mentally ill population.
