The Wellness Coordinator is responsible for coordinating care and wellness programs to support the physical, emotional, and mental well-being of residents. This role involves scheduling Caregivers and Certified Medication Aides (CMA's), working closely with the healthcare team to ensure smooth operations in resident care, and providing support to other departments as needed.

DUTIES AND RESPONSIBILITES

Resident Care Coordination:

  • Schedule and coordinate caregivers and CMA's to provide efficient resident care, ensuring adequate staffing levels for each shift.
  • Collaborate with the care team to manage the care schedules for residents, ensuring continuity of care and timely services.
  • Conduct wellness assessments and develop personalized plans in partnership with nursing and care staff.
  • Maintain communication with families and caregivers to ensure they are informed about residents’ wellness plans and progress.

Team Collaboration:

  • Work closely with caregivers, CMA's, and other healthcare professionals to provide a coordinated approach to resident care.
  • Assist other departments as needed, ensuring that the overall community operations run smoothly.

Administrative Responsibilities:

  • Oversee staff schedules to ensure adequate coverage for care and CMA support, adjusting as necessary to meet resident needs.
  • Maintain accurate records of resident wellness assessments, program participation, and health trends.
  • Prepare reports on wellness outcomes and program effectiveness for leadership review.

QUALIFICATIONS FOR THE JOB

  • Experience in care coordination, wellness, or a similar role in a healthcare or senior living environment.
  • Strong organizational and communication skills, with the ability to manage schedules and work with interdisciplinary teams.
  • Proficiency in scheduling software and wellness management tools.
  • CNA certification
  • CMA certification
