Position Summary
The Health & Wellness Coordinator manages daily healthcare operations to ensure residents receive high-quality, individualized care that evolves with their needs.
This role partners closely with community leadership and the clinical team to maintain compliance, support staff, and promote resident dignity and wellbeing.
Key Responsibilities
Clinical Coordination
  • Oversee day-to-day resident healthcare services
  • Assist with resident assessments and care plan updates
  • Monitor changes in resident condition and communicate appropriately
  • Support medication management and documentation compliance
  • Ensure care is delivered with dignity and respect
Team & Operational Support
  • Partner with caregivers and medication aides
  • Provide clinical guidance and support to staff
  • Help ensure appropriate staffing and coverage
  • Promote a positive, resident-centered care culture
Quality & Compliance
  • Support state regulatory compliance and survey readiness
  • Maintain accurate clinical records
  • Monitor quality indicators and follow up on concerns
  • Collaborate with the Executive Director and Health & Wellness leadership
Qualifications
Required
  • Active LPN license (Maryland)
  • 1-2 years of relevant clinical experience
  • Passion for working with seniors
  • Strong communication and organizational skills

Preferred
  • Assisted Living or Memory Care experience
  • Previous Resident Care Coordinator or Wellness Nurse experience
  • Experience with care planning and clinical documentation systems
