Job description

We are currently looking to add a qualified Wellness Nurse to the amazing team at our Paradise Valley Senior Living!

This position supports the overall wellness of residents by assisting with health assessments, medication management, care planning, and coordination with families and healthcare providers. The Wellness Nurse also works closely with caregivers and other team members to promote a safe and healthy environment.

Responsibilities:

  • Monitor changes in residents' conditions and communicate concerns to appropriate team members and families.
  • Administer medications and treatments in accordance with state regulations and community policies.
  • Maintain accurate and timely documentation in electronic medical records (EMR).
  • Provide training, supervision, and support to caregivers regarding proper care techniques and safety.
  • Respond to medical emergencies and coordinate appropriate interventions.
  • Communicate with physicians, pharmacies, and other healthcare providers as needed.
  • Participate in care plan meetings and contribute to interdisciplinary team discussions.
  • Promote resident independence, dignity, and choice in alignment with our mission of person-centered care.

Requirements:

  • Must have a valid LPN/RN in the state of Arizona
  • Finger print clearance card
  • CPR First Aid
  • One year of nursing experience, preferably in an assisted living environment
  • Proficiency with computers and Microsoft Office programs
  • Must enjoy working with the senior population
  • Must be able to maintain mental alertness, attention, and concentration for necessary periods
