The role of Physical Therapist is an onsite position primarily dedicated to home care. This means you will interact with patients in the comfort of their own homes, providing a personal touch that fosters strong therapeutic relationships. Our work setting emphasizes a balance between clinical expertise and the comfort of home, allowing you to apply your skills in a familiar environment for both you and your patients. You will work a consistent schedule, Monday to Friday from 2 PM to 8 PM, allowing for a structured yet flexible work life.

• Deliver high-quality, patient-centered care by thoroughly managing the entire therapeutic process for a dedicated caseload of patients, liaising seamlessly with physicians and the interdisciplinary care team.
• Conduct comprehensive assessments of patients to understand their physical capabilities and limitations, actively participate in formulating individualized care plans aimed at achieving set therapeutic goals.
• Implement and monitor treatment interventions while ensuring compliance with established legal and ethical frameworks.
• Utilize therapeutic methods and advanced technologies to drive improvement in patients' physical health, adjusting methodologies as required based on ongoing evaluations.
• Guide patients and their families in the effective use of exercises, therapies, and any necessary equipment to extend their rehabilitation efforts beyond therapy sessions.
• Maintain accurate documentation of patient progress and modify treatment plans in collaboration with healthcare team members to ensure the best possible patient outcomes.
• Prioritize treatment workloads efficiently, assessing daily staffing needs to delegate responsibilities appropriately.

Your primary focus will be on treating a diverse patient population, including individuals recovering from surgeries, managing chronic pain, or experiencing mobility impairments. You will provide therapeutic services that cater to unique patient needs, ranging from pediatric to adult care. Ensuring a personalized approach tailored to each patient’s specific conditions and goals will be critical in promoting positive health outcomes.

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Current and valid license to practice Physical Therapy in California.
• Basic Cardiac Life Support (BCLS) certification required within the first 30 days of employment.
• Commitment to lifelong learning and practice improvement.

• Previous experience in home health or community-based settings is highly desirable but not required.
• Familiarity with rehabilitative tools and technology commonly used in physical therapy.
• Diverse experience treating various patient demographics and conditions.
