Position Overview
We are seeking a full-time Nurse Practitioner to join a collaborative primary care clinic in West Lafayette, Indiana. This role offers the opportunity to provide comprehensive patient care in a multidisciplinary setting that includes pharmacy support, health coaching, and behavioral health services. The ideal candidate is comfortable managing patients with chronic conditions, including weight management and GLP-1 medication therapy.
New Nurse Practitioner graduates may be considered for this opportunity.


Schedule

  • Preferred schedule: Tuesday through Friday, four 10-hour shifts (40 hours/week)
  • Clinic Hours:
    • Monday-Thursday: 7:00 AM - 7:30 PM
    • Friday: 7:00 AM - 5:00 PM

Key Responsibilities

  • Conduct pre-employment and annual wellness examinations.
  • Evaluate, diagnose, and treat acute and chronic medical conditions using evidence-based clinical guidelines.
  • Develop and implement individualized treatment plans in collaboration with the supervising physician.
  • Perform preventive health screenings and wellness initiatives.
  • Assess and treat workplace injuries and illnesses.
  • Review clinical cases with the Medical Director and appropriate stakeholders as needed.
  • Oversee employee respirator fit testing and hearing conservation testing performed by clinic staff.
  • Educate patients on disease prevention, healthy lifestyle choices, and treatment plans.
  • Order, interpret, and act upon diagnostic testing.
  • Monitor patient progress and adjust care plans as appropriate.
  • Coordinate care with multidisciplinary providers and make referrals when necessary.
  • Accurately document patient encounters within the electronic medical record.
  • Support overall clinic operations as needed.

Required Qualifications

  • Board Certified Nurse Practitioner with an active state license.
  • Minimum of 1 year of experience in family medicine, internal medicine, or urgent care (new NP graduates may also be considered based on client preference).
  • Current BLS certification.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with healthcare professionals and organizational leadership.
  • Commitment to delivering patient-centered, compassionate care.
  • Ability to work in a fast-paced clinical environment while maintaining high ethical and professional standards.
  • Proficient with electronic medical records and clinical documentation.

Preferred Qualifications

  • DEA license.
  • Experience with weight management programs and GLP-1 medications.
  • Occupational health or employer-based clinic experience.
