Telehealth Nurse Practitioner (NP) – Complex Care | Spanish Required

Position Overview

We are seeking an experienced Nurse Practitioner to provide meaningful care to medically complex patients throughout New Mexico, primarily through telehealth and remote-care technologies.

This role offers the opportunity to join a collaborative, interdisciplinary team focused on prevention, early intervention, care coordination, and improved patient outcomes. The Nurse Practitioner will provide clinical care and ongoing management while working closely with patients, families, caregivers, primary-care providers, specialists, and health-plan partners.

Client serves highly complex populations, including Medicaid and Medicare/Medicaid Dual Eligible members. The position emphasizes clinical autonomy, strong care coordination, and innovative approaches to home-centered healthcare.

Professional proficiency in speaking, reading, and writing Spanish is required.

Key Responsibilities

• Provide clinical care and ongoing patient management through telehealth and remote-care technologies.
• Manage medically complex Medicaid and Dual Eligible patient populations.
• Assess patient needs and collaborate with physicians and interdisciplinary team members to develop and execute individualized care plans.
• Work closely with patients, families, caregivers, primary-care providers, specialists, and health-plan partners.
• Identify changes in patient health and intervene early when possible to help prevent unnecessary emergency-department visits and hospitalizations.
• Support patients through hospital admissions, discharges, and transitions back to the home setting.
• Coordinate care and help ensure patients receive appropriate clinical, ancillary, and community-based services.
• Participate in advance-care planning and goals-of-care discussions with patients and families.
• Participate in a shared clinical triage and support model.
• Support quality initiatives through accurate clinical documentation related to HCC coding, risk adjustment, STAR and HEDIS measures, and closure of quality gaps.
• Contribute to clinical best practices and help shape client's growing and innovative care model.

Required Qualifications

• Master's degree or higher.
• 1+ year of telemedicine/telehealth care experience.
• Full professional proficiency in Spanish, including speaking, reading, and writing.
• Appropriate Nurse Practitioner licensure/authorization necessary to provide care to patients in New Mexico.

Ideal Candidate

The ideal candidate is an experienced Nurse Practitioner who is comfortable providing care through telehealth and enjoys working with patients who have significant medical and social complexity.

Experience managing patients longitudinally in a virtual environment is highly valued, particularly when that experience includes chronic-disease management, medication management, care coordination, follow-up, and early intervention. However, the stated requirement for this position is 1+ year of telemedicine care experience, rather than a specified number of years of virtual longitudinal care.

The successful candidate should also be comfortable communicating with Spanish-speaking patients and families and collaborating across an interdisciplinary care team.
