WEEKEND REGISTERED NURSE RN - HOME HEALTH

$7,500 Sign-On Bonus

12-hour shifts
Saturday and Sunday required
One weekday of your choice

St. Joseph and Elkhart Counties
Must be willing to travel within coverage area

As a Home Health RN with Elara Caring, you provide one-on-one care in the patient's home. You manage your own caseload, build meaningful relationships, and see real progress in real time.

WHAT YOU WILL DO

Manage and coordinate your own home health caseload
Complete Start of Care, follow-up, and discharge visits
Perform comprehensive patient assessments
Develop and update individualized plans of care
Complete OASIS documentation accurately and timely
Educate patients and families on medications and disease management
Monitor patient progress and report changes to physicians
Collaborate with interdisciplinary care team members
Ensure documentation meets compliance and regulatory standards

WHAT YOU NEED

Active, unrestricted RN license
At least 1 year of nursing experience
Willingness to travel within the coverage area
Reliable vehicle, valid driver's license, and auto insurance
Home Health experience preferred
