Home Health Registered Nurse (RN)

Weekend Schedule | Lafayette, IN | Flexible Options Available

Weekends only + weekdays free. Provide 1:1 patient care in the home — no call lights or 6-patient assignments. New to home health? We provide training and ride-along support.

We are hiring a Weekend Home Health RN in the Lafayette area. This role is ideal for nurses who prefer fewer weekday obligations, want a consistent routine, or are balancing family, school, or a second job.

What You’ll Do

As a Home Health RN, you’ll care for patients in their homes and help them safely manage their health conditions.

Responsibilities include:

  • Completing start-of-care and routine nursing visits

  • Assessing patient condition and developing care plans

  • Providing medication and disease-management education

  • Monitoring progress and preventing hospital readmissions

  • Coordinating care with physicians, therapists, and care team members

  • Identifying changes in condition and communicating appropriately

  • Documenting visits in the electronic medical record

  • Maintaining HIPAA and patient safety standards

Training & Support

You will not be sent out unprepared. We provide:

What We’re Looking For
  • Active, unrestricted RN license

  • 1+ year of nursing experience (hospital, SNF, rehab, clinic, etc.)

  • Willingness to work independently in the field

  • Reliable vehicle, valid driver’s license, and auto insurance

  • Ability to travel within the Lafayette coverage area

  • Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

Preferred (not required):

  • Home Health or Hospice experience

  • HomeCare HomeBase (HCHB) experience

  • ASN or BSN
