As a Home Health Registered Nurse (RN) on our per diem team, Home Health Registered Nurse (RN) - Brisben (Chenango), you will play a vital role in ensuring the well-being of our patients and providing exceptional care and supervision to our home health aides.
Key Responsibilities:
โ€ข Assess new patients and develop personalized care plans for aides to follow
โ€ข Orient aides to new patient cases and care expectations
โ€ข Conduct routine supervisory visits for caregivers within your assigned county
โ€ข Perform recertification assessments for patients every six months
โ€ข Ensure timely and accurate documentation for all assessments and care plans
Qualifications:
โ€ข Active, unencumbered NYS Registered Nurse (RN) license
โ€ข Previous home care experience preferred
โ€ข Reliable transportation to travel within assigned areas
