SUMMARY: As a Registered Nurse you provide the initial and ongoing assessment of patients, ensuring appropriate initiation of care plans, services/levels, and OASIS assessment information that accurately reflects the patient’s clinical, functional, and psychosocial needs.

Weekend Schedule: Full-time weekend position working 30 hours per week on either Friday–Sunday or Saturday–Monday (three 10-hour shifts). Enjoy pay for 40 hours per week while receiving full-time benefits.

Sign on Bonus: Up to $15,000

Location: Serving Counties Wayne, Seneca, Ontario and Yates

Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience to be considered:

Education: New York State Licensed RN with a minimum of one year of experience as a clinical nurse in a hospital or home care agency or BSN.

Experience: A minimum of six (6) months experience in hospital or homecare. Minimum of one-year experience in community health. VNS experience preferred.

Licensure: New York State Licensed RN

Key Responsibilities

  • Conduct comprehensive patient assessments, including OASIS documentation, and develop individualized care plans.
  • Coordinate care with patients, families, physicians, and the interdisciplinary team to ensure quality outcomes.
  • Maintain accurate, timely clinical documentation in compliance with state, federal, and organizational standards.
  • Educate patients and caregivers while connecting them with appropriate community and healthcare resources.
  • Communicate changes in patient status and collaborate with the care team to support continuity of care.
  • Manage time, resources, and productivity effectively while delivering high-quality, cost-conscious care.
  • Participate in on-call, evening, or weekend visits as needed to support patient care.
  • Maintain professional competency through continuing education and perform other duties as assigned.

Salary: $73,652 - $103,854
