This role is fully remote, with the flexibility to work from home while maintaining regular communication with the hospice care facility and other team members. While the work schedule is flexible, you may be required to provide evening and weekend availability to meet the unique needs of clients and their families during evenings and weekends.

• Conduct thorough psychosocial assessments to identify clients' emotional and mental health needs, strengths, and available resources.
• Deliver individual, family, and group therapy sessions utilizing therapeutic modalities such as cognitive-behavioral therapy and trauma-focused therapy.
• Collaboratively develop and implement personalized treatment plans, setting clear goals and interventions tailored to each client.
• Coordinate closely with psychiatrists, psychologists, and the broader treatment team to ensure comprehensive, client-centered care.
• Provide timely crisis intervention, conducting risk assessments and implementing safety plans as necessary.
• Maintain accurate and compliant documentation in line with ethical, legal, and organizational standards.
• Facilitate engaging group therapy sessions and educational classes to enhance client knowledge and coping strategies.

Your primary focus will be on clients facing the complexities of hospice care, including their families. This population often grapples with grief, anticipatory loss, and various mental health concerns such as anxiety and depression related to their circumstances. You will play a crucial role in providing emotional support, helping clients navigate their feelings, and facilitating discussions about life transitions with empathy and sensitivity.

• Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Texas.
• A minimum of two years of post-graduate experience in a mental health or outpatient setting, preferably with exposure to hospice or palliative care environments.
• Demonstrated expertise in evidence-based therapeutic modalities and a commitment to continuous professional development.
• Outstanding interpersonal and communication skills, with a strong ability to assess client needs and build rapport.

• Experience working specifically with terminally ill patients and their families, understanding the unique challenges they face.
• Familiarity with electronic health record systems to maintain accurate patient documentation and facilitate coordinated care.
• Advanced skills in crisis intervention and safety planning.
