This position is fully remote, allowing for flexibility and work-life balance while enabling the clinician to connect meaningfully with clients in a hospice care setting. You will interact with a multidisciplinary team, participate in virtual meetings, and utilize digital tools to coordinate care effectively. • As a clinician, you will engage with clients both virtually and in the community, delivering individualized support to those grappling with significant mental health issues.
• Participate in team-based care, working collaboratively to meet common objectives and advancing mutual goals.
• Conduct brief, evidence-based screenings and arrange for more in-depth assessments when necessary.
• Create, implement, and monitor integrated care plans that ensure clients receive comprehensive services tailored to their needs.
• Provide direct therapeutic services including individual, group, and family therapy, adjusting interventions based on clients’ evolving needs.
• Assist clients in crisis situations by employing intervention techniques to mitigate acute symptoms and ensure their safety.
• Maintain ethical standards and uphold a code of conduct as per professional guidelines.
• Document and report all client interactions accurately and timely to ensure continuity of care.
• Participate actively in team meetings and committees, sharing insights and strategies for enhanced service delivery.
• Provide on-call coverage during specified times to support clients needing immediate assistance.

• A required Bachelor's Degree in Social Work or a related field.
• Hold an active Licensed Clinical Social Worker (LCSW) license.

• Proficient in using technology and various software systems necessary for documentation and client management.

• Experience working in hospice care environments, particularly with clients facing terminal illnesses.
