We are backed by leading healthcare and AI investors and headquartered in El Segundo, CA. The Role Cover weekend tele visits for home health patients in the SoCal/OC region. As a per diem weekend tele NP, you'll conduct remote assessments on Saturdays and/or Sundays, act on Circadia platform alerts, and ensure continuity of care for high-risk patients between weekday care team touchpoints. What You'll Do
  • Weekend remote patient care: Conduct follow-up and urgent telehealth assessments on flagged patients; adjust plans of care as needed; coordinate with on-call providers and care teams.
  • Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and intervene on weekend alerts to prevent unnecessary ED visits or hospitalizations.
