As a Weekend In-House Nurse Practitioner, you will be supporting the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

Job Requirements:

  • Active Illinois Nurse Practitioner License (unencumbered).

  • Master's Degree in Nursing from an accredited program.

  • Strong clinical background in primary care and preventive medicine.

  • Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.

  • Ability to engage and communicate effectively with elderly and chronically ill patients and their families.

  • Ability to maximize onsite time to develop robust caseloads

  • Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Key Responsibilities:
As the Weekend In-House Nurse Practitioner, you will provide essential care and leadership within the facility, including:

  • Ensuring continuity of care by communicating with the primary In-House Nurse Practitioner

  • Conducting new admission evaluations and monthly stability visits for long-term residents

  • Performing comprehensive physical examinations and evaluating changes in patient conditions

  • Prescribing medications and treatments as needed

  • Developing patient care policies and facilitating referrals to specialists

  • Coordinating advanced care planning and discharge visits

  • Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups

  • Supporting the physician-patient relationship and addressing gaps in care
