Job description

Day Program Nurse (LPN / RN) - DTA & Community Program

We are seeking a compassionate and dependable Day Program Nurse to support the health and safety of adults participating in our Day Treatment for Adults (DTA) and Group Supported Employment (GSE) programs within a DDD-certified environment.

The ideal candidate will have experience working with individuals with intellectual and developmental disabilities, behavioral health needs, or community-based programs. This role focuses on health oversight, medication support, wellness monitoring, and ensuring members remain safe and supported while participating in daily activities and community outings.

This position is ideal for a nurse who enjoys working outside of traditional clinical settings and supporting individuals in a person-centered, community-based environment.

Key Responsibilities

Health Oversight & Safety

Provide general health monitoring and nursing oversight for members attending the day program
Monitor members for changes in health status or medical concerns
Perform vital sign checks and health assessments when needed
Provide basic first aid and respond to minor medical concerns

Medication Management

Assist with medication administration as ordered
Maintain Medication Administration Records (MAR) and medication cards
Monitor medication compliance and documentation accuracy
Coordinate medication-related concerns with physicians, pharmacies, and leadership

Nursing Support

Monitor members with chronic health conditions such as diabetes, seizures, or mobility limitations
Check blood sugars when required
Administer insulin if ordered
Ensure medical supplies and medical bags are stocked and ready for outings

Documentation & Compliance

Maintain accurate health documentation and medical records
Support DDD, AHCCCS, and HCBS compliance standards related to health services
Monitor incident reports and assist with health-related documentation in systems such as Therap or AxisCare

Community Participation & Outings

Accompany members on community volunteer outings and activities
Carry a medical bag and provide health support when needed during outings
Support members in participating safely in community activities and employment readiness programs

Team Collaboration

Work closely with Direct Support Professionals (DSPs), job coaches, and program leadership
Provide guidance to staff regarding health needs and safety practices

Required Skills & Qualifications

Active Arizona LPN or RN license
Knowledge of developmental disabilities and DDD service environments
Strong communication skills when working with individuals with disabilities and multidisciplinary teams
Ability to respond to medical or behavioral needs with compassion and professionalism
Ability to maintain accurate health documentation

Preferred Qualifications

Experience working in DDD programs, adult day programs, behavioral health, or group homes
Familiarity with documentation systems such as Therap or AxisCare
Current CPR / First Aid certification
Valid Fingerprint Clearance Card
Article 9 certification (or willingness to obtain)
Bilingual (Spanish/English) is a plus
