Description
Job Summary:The Direct Support Professional is responsible for supporting female individuals with intellectual and developmental disabilities in a residential setting located in Silver Spring, MD as they exercise control over the things in their life that are important to them, to the degree that they desire.
Entry rate of $18.40 per hour, will increase to $20.00 at the successful completion of the introductory period and required DDA and organization training.
Duties and Responsibilities:
Direct Support
โ€ข Facilitates a customized community-driven schedule that focuses on the individuals' wishes and goals.
โ€ข Assists the individuals to engage in their community to develop natural support and peer relationships.
โ€ข Assists with independent living skill development in the individual's home, as determined by the individual's wishes and goals.
โ€ข Working with individuals' families and teams to ensure concerns are successfully resolved.
โ€ข Ensuring individuals can access their community by providing transportation to and from planned activities.
โ€ข Caring for the emotional and physical needs of the individuals, according to their level of independence through positive interactions.
โ€ข Participates in medical appointments.
โ€ข Ensures homes are maintained in a clean and safe manner.
โ€ข Review and be familiar with nursing medical status and assessments of persons we support.
โ€ข Working cooperatively with the Residential Coordinator to ensure staff and team members contribute to an individual's Person-Centered Plan (PCP) by ensuring written goals are implemented and documented daily.
Administration
โ€ข Attending meetings with families, as scheduled.
โ€ข Assuring the privacy of the individual's information by appropriately limiting disclosures of protected information.
โ€ข Store, handle, and share protected information in accordance with CHI policies and HIPAA regulations.
โ€ข Document all incidents, PCP goals, behavior plan collection, and other documents as required.
โ€ข Providing ongoing, timely communication to the assigned Residential Coordinator for all items of importance as it relates to the care and welfare of the individual supported.
โ€ข Reviewing and being familiar with nursing medical status and assessments of individuals supported
Requirements
Required Qualifications:
โ€ข High School Diploma or GED equivalent
โ€ข Valid Driver's License
Preferred Qualifications:
โ€ข Minimum 1 year working with individuals with intellectual and developmental disabilities
Physical Requirements:
โ€ข Prolonged standing, leaning, walking, bending, and lifting periods.
โ€ข Must be able to lift to 50 pounds at times.
