Job description

PRN LPN/RN Shift Lead โ€“ Memory Care PRN | Overnight, Weekend & Holiday Coverage

Join a small, boutique memory care residence founded by a geriatric psychiatrist and intentionally designed to provide highly personalized care for just 16 residents.

Arbor Place is seeking compassionate, dependable LPNs and RNs to join our PRN nursing team. We have a particular need for nurses interested in overnight, weekend, holiday, vacation, as well as other open-shift coverage as part of a pool of exceptional nurses.

As the licensed nurse and shift lead on duty, you will administer medications, monitor residents for changes in condition, coordinate and support caregivers, communicate with families and healthcare providers as appropriate, and help ensure that each resident receives attentive, individualized care.

Because we care for only 16 residents, our team and LPN/RN has the opportunity to truly know our residents, understand their individual needs, and work closely with the caregivers and families who know them best.

Schedule

This is a PRN/as-needed position with a particular need for overnight, weekend, and holiday coverage, as well as occasional coverage for vacations, call-outs, and other open shifts. We have an exceptional,long-tenured team that collaborates and enjoys working together. We have very low turnover.

Availability for every type of shift is not required. We welcome nurses who can reliably provide coverage for one or more of these needs.

Responsibilities
  • Serve as the licensed nurse and shift lead during the assigned shift.
  • Administer medications and treatments in accordance with physician orders, nursing standards, and Arbor Place policies and procedures.
  • Monitor residents for changes in condition, behavior, mobility, comfort, appetite, and overall well-being.
  • Recognize and respond appropriately to changes in resident condition and communicate significant concerns to the Director of Nursing and other appropriate healthcare providers.
  • Communicate professionally and compassionately with residents' families regarding relevant changes, concerns, and updates.
  • Provide direction, guidance, and support to caregivers during the shift.
  • Help ensure that individualized service and care plans are followed.
  • Complete medication, treatment, observation, incident, and other required documentation accurately and promptly.
  • Communicate important resident information at shift change to support continuity of care.
  • Respond and proactively address changes in condition, and other resident needs in accordance with established clinical protocols.
  • Collaborate with the Director of Nursing, caregivers, physicians, pharmacy, hospice, and other healthcare providers as appropriate.
  • Help maintain a safe, calm, respectful, and resident-centered environment.
Qualifications
  • Current, active Maryland LPN or RN license in good standing.
  • Current CPR/BLS certification.
  • Experience with medication administration and direct nursing care.
  • Ability to recognize, document, and respond appropriately to changes in resident condition within the scope of licensure.
  • Ability to exercise appropriate independent judgment while serving as the licensed nurse on duty.
  • Ability to read, understand, and follow physician orders, medication records, service plans, policies, and clinical documentation.
  • Strong communication skills and the ability to work effectively and compassionately with residents, families, caregivers, and healthcare providers.
  • Dependable and comfortable working independently within an established clinical care structure.
  • Memory care, assisted living, geriatrics, longโ€‘term care, or home health experience preferred.
Essential Requirements

The successful candidate must be able to perform the essential functions of the position, with or without reasonable accommodation.

The position requires regular walking and standing throughout the community and may require bending, reaching, pushing, pulling, and assisting with resident mobility, transfers, or emergency situations as appropriate to the nurse's role.

The nurse must be able to communicate changes in a resident's condition or needs clearly and accurately, follow written and verbal instructions, exercise appropriate judgment in urgent situations, and work collaboratively with residents, families, caregivers, and other healthcare professionals.

This position involves working directly with older adults and those with dementia and other medical or cognitive conditions and may involve exposure to infectious diseases, blood or body fluids, and residents experiencing behavioural symptoms associated with dementia. Established infectionโ€‘control, safety, confidentiality, resident-rights, and emergency procedures must be followed at all times.
