Job Summary:

The Registered Nurse (RN) is responsible for delivering comprehensive nursing care to patients in a post-acute care setting. This role includes assessing patient health, developing and implementing nursing care plans, and collaborating with interdisciplinary teams to provide high-quality patient care.

Key Responsibilities:
  • Conduct detailed assessments of patient health conditions and monitor ongoing health status.
  • Implement individualized care plans in coordination with healthcare providers.
  • Administer medications and treatments as prescribed, monitoring for side effects or adverse reactions.
  • Provide patient and family education on disease processes, treatment protocols, and self-care.
  • Ensure accurate and timely documentation of patient information, including vital signs, progress notes, and medical history.
  • Collaborate with physicians, therapists, social workers, and other team members to achieve the best patient outcomes.
  • Participate in discharge planning, ensuring a smooth transition to home care or other appropriate settings.
