Job description

Village Caregiving - Cape Cod, MA

Village Caregiving - Cape Cod, MA is looking for a PRN Registered Nurse to join our home care team! This role includes visiting clients in and around Buzzards Bay, MA to complete health assessments, document medical history, and develop personalized care plans for caregivers. The RN may also assist with training new caregivers as needed. This position offers a flexible schedule and is a great opportunity for a nurse who enjoys working independently while making a difference in the home care setting.

Responsibilities
  • Visit patients in their homes for one-on-one visits
  • Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
  • Assess patients and their environment to develop a care plan in order for the caregiver to have a blueprint to know how to safely and competently take care of our patients
  • Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient's quality of life
  • Provide effective communication to patients/clients, their family members, team members, and other health care professionals
  • Fill out nurse assessment documents on a supplied tablet the same day as assessment. Upload assessment to CRM.
  • Keep timely communication with the director of operations on schedule for upcoming and current assessments.
  • Notify the director of operations at the end of each pay period list of assessments completed.
Qualifications
  • Graduate of an accredited diploma, associate, or bachelor's school of nursing, or vocational nursing
  • Current state license as an RN
  • Proof of current CPR certification
  • Compassionate, friendly demeanor
  • Excellent communication skills
  • Current driver's license and reliable transportation
  • Ability to work a flexible schedule and travel locally
