Overview
The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.
This is an every weekend position 7:00 AM-5:30 PM on site in our Farmingdale office located at 110 Bi-County Boulevard.
Job Details
Our Home Care Transition Nurses:
  • Establish and maintain professional relationships with key physician groups and referral sources.
  • Act as Liaison to referral sources on behalf of Catholic Home Care.
  • Identify potential market growth opportunities and program expansion.
  • Serve as a Customer service representative and act in a professional and supportive manner at all times.

Requirements:
  • Current New York State Registered Nurse licensure
  • 2 or more years of home care experience preferred
  • Customer Service and Marketing skills required

Posted Salary Range
USD $55.00 - USD $65.00 /Hr.
