Pay Rate: Min $114,545.60 Mid $143,187.20Position Summary
Provides pharmaceutical care to critically ill or ED patents according to expert judgment and LRH policies and procedures. Work within the scope of Pharmacy Laws and Standards of Practice to maximize benefit to the patient while minimizing associated costs. Functions include rounding as a member of the multidisciplinary critical care team, if applicable, and to provide pharmacotherapeutic management for all ICU/ED patients, responding to resuscitation events, and coordinating the development and implementation of drug therapy protocols to maximize benefits of drug therapy. He/she must identify opportunities for and contribute to the improvement of quality, safety, and costs, as well as patient, customer, and employee satisfaction.

Standard Work Duties: Critical Care Clinical Pharmacy Specialist
  • Actively participates in pharmacy team development and accomplishing department goals and objectives. Remains current with all LRH mandatory annual education.
  • Provides comprehensive pharmaceutical care (pharmacokinetic monitoring, nutritional support, patient-specific dosing of medications with consideration of age, renal and hepatic insufficiency, underlying conditions, provision of drug information, etc.) for the critically ill/ED patient in a manner consistent with consensus guidelines and evidence-based medicine.
  • Evaluates drug therapy for appropriate indications, dosing, drug interactions, and drug allergies. Monitors the patient's pharmacotherapeutic regimen for effectiveness and adverse drug events/reactions and intervenes as needed. Reports adverse drug events/reactions in appropriate institutional system(s) and assists in determining root cause(s) and any potential improvements in the medication use system.
  • Actively participates as a member of the multidisciplinary critical care/ED team (physicians, nurses, other pharmacy staff, etc.) to provide cost-effective and efficient medication and resource utilization for all critically ill/ED patients. Participates in multidisciplinary patient care rounds when applicable.
  • Participates in critical care/ED pharmacotherapy research and/or quality improvement efforts with a focus on improvement in patient outcomes.
  • Coordinates the development, implementation, education, and assessment of evidence-based pharmacotherapy protocols.
  • Provides consultation to hospital committees, such as Pharmacy and Therapeutics (P&T), when critical care or ED pharmacotherapy issues are discussed. Assists with monograph development or formulary reviews when needed.
  • Provides education to the critical care/ED team members regarding current drug-related policies and procedures as well as other pharmacotherapy topics as necessary. Provides education to pharmacy staff in the form of a presentation or in-service at least annually or at the direction of the Pharmacy Clinical Coordinator. Participates in the education of pharmacy residents and students, and overall residency program requirements and activities as outlined in the LRH Pharmacy Residency Program Manual and in accordance with the ASHP Accreditation Standard.
  • Identifies how drug costs may be minimized through appropriate use of drugs in the critical care/ED areas and through implementation of cost-containment measures.
  • Organizes job functions and work assignments to be able to effectively complete assignments or projects within established time frames.

Other information:
Licenses Essential: Eligible for Florida Pharmacist license.
Licenses Preferred: Active Florida Pharmacist license.
Certifications Preferred: Board Certified Pharmacotherapy Specialist or Board Certified Critical Care Pharmacist.
Experience Essential: Successful completion of an ASHP-accredited PGY1 residency with at least two years of critical care/ED clinical experience, or equivalent.
Experience Preferred: Successful completion of a PGY2 Critical Care or Emergency Medicine residency, or have at least three to four years of critical care/ED clinical experience.
