Job Description

You will be responsible for providing high-quality nursing care to patients in the community. You have clinical skills and knowledge of wound management, infusion therapy, palliative care, and chronic disease management.

  • Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of the health care team
  • Ensure that nursing care provided to our patients is based on best practices to achieve successful outcomes
  • Continually assess, anticipate, identify and respond to new and changing needs of our patients
  • Travel within your region between patients’ homes
Qualifications
  • Minimum two years nursing experience (Community Nursing experience is ideal) 
  • Registered and in good standing with the College of Nurses of Ontario (CNO)
  • Ability to work independently in the community
  • Proficiency in English, both verbal and written required (additional languages an asset)
  • Valid Ontario driver's license and reliable vehicle
  • Experience with infusion therapy, mental health, palliative care, and wound care
  • Excellent patient assessment and teaching skills
