In Conjunction With The Medical Plan Of Care, Independently Perform Systematic And Continuous Assessment Of The Physical, Emotional, Clinical, And Psychosocial Needs Of Assigned Patients And Families; Plan And Implements Appropriate Interventions And Evaluates Effectiveness And Adjusts Plan As Indicated In A Timely Manner In The Provision Of Safe And Effective Care.
