Job Description:
The Home Health Weekend Nurse provides one on one care and skilled services for patients in their own home, contributing to the individual's overall quality of life.
Weekend Nurses primarily perform admissions for all levels of patient care including Infusion, Wounds. Providing education and resources to patients and caregivers allowing them to manage their conditions and remain in their own home. This position will provide In-Home evaluation, direction and application of Skilled Nursing Services to complement the patient's overall Plan of Care, including notifying Providers of changes in condition and recommending a higher level of care when warranted.
Education:
  • Required: Graduate of an accredited nursing program

Experience:
  • Preferred: Minimum one (1) year experience as professional nurse within the last three (3) years.

Skills
  • Ability to communicate effectively while working in the field.
  • Ability to work in a team structure and collaborate with coworkers to care for the patient.
  • High level of organization, responsibility and self-management.
  • Ability to work independently utilizing high level of critical thinking and problem solving skills.
  • Ability to provide reliable transportation as required by nature of position.

Licensure/Certification/Registration:
  • Required: Active Driver's License
  • Required: RN License active in the state of Missouri
  • BLS must be obtained within 90 days of hire
