Pediatric Home Service is looking for skilled and compassionate Nurse Supervisor to lead our team in the Topeka, KS area! This position is responsible for coordinating all aspects of client care which includes clinical support, consultation and educational enrichment for staff, clients and families while supporting all caregivers to ensure their professional success in the homecare setting.

THE POSITION:

  • Schedule: Full Time, Monday-Friday, 8 am- 5 pm, on site with frequent regional travel
  • License Requirements: Must be a Registered Nurse licensed in good standing with applicable state board of nursing

The Nurse Supervisor is responsible for coordinating all aspects of client care with a focus on optimal clinical outcomes. This includes clinical support, consultation, and educational enrichment for staff, clients, and families, supervision of caregivers, development and delivery of contracted services, implementation of policies and procedures, and collaboration on marketing/outreach efforts. This position requires frequent partial to full day travel regionally around branch office and occasional travel to other branches, corporate office, or other locations deemed necessary. Through company or personal vehicle, self-transportation is required for this position.

  • Supervises and evaluates direct patient caregivers
  • Assesses the learning needs and competencies of direct patient caregivers and provides education as needed
  • Participates in the coordination and continuity of orientation and training programs; Practices and promotes professional development of staff, peers, and self
  • Provides a challenging and fulfilling work environment by empowering staff to provide a high level of customer service
  • Participates in recruitment including sourcing, interviewing, orientation, and training as directed by Branch Administrator
  • Completes initial and on-going comprehensive assessments including OASIS assessments at appropriate time points and as directed
  • Initiates the Plan of Care and revises as needed to include appropriate preventative and rehabilitative nursing cares and procedures including use of Home Health Aide level of care
  • Initiates or participates in care planning and coordinating for clients including potential admissions, clients transferring to another care setting or level or care and clients planning to discharge from services entirely
  • Monitors and evaluates client outcomes; ensures that staff, clients, and families have resources for effective and efficient services
  • Serves as a communication liaison and care coordinator to staff, clients, physicians, and families including providing updates to changes in condition or needs
  • Participates in continuous quality improvement and management programs implementing and communicating corrective action plans and interventions
  • Maintains relationships with Case Managers, Hospital staff and Physicians, Families, and Clients to promote awareness of company services and increase referrals
  • Occasionally attend client shifts to assess, problem solve, provide specialized care, and facilitate improving nurse/client/family relations
  • Participates in rotation to provide after-hours phone support to staff and clients
  • Completes and maintains clinical/progress notes, documentation, files, and records as required
  • Provides direct patient care within the scope of their license as requested or needed
  • Other related duties as assigned

THE QUALIFICATIONS:

  • Must be a Registered Nurse licensed in good standing with Kansas state board of nursing
  • Two (2) years of experience working as a registered nurse; Pediatric and home care experience preferred
  • Self-transportation is required for this position through use of company or personal vehicle
