Home Care Transition Nurse

Make a meaningful difference in patients' lives by helping them transition safely and successfully from Good Samaritan Hospital in West Islip.

Catholic Home Care is seeking a compassionate, experienced Registered Nurse to join our team as a Home Care Transition Nurse. In this highly visible role, you'll serve as a trusted clinical liaison, collaborating with physicians, patients, families, and healthcare partners to ensure seamless care transitions while expanding access to high-quality home care services.

The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.

  • 5 x 8 hour shifts (9:30AM to 5:30PM) - Rotating weekends and holidays
  • Great healthcare benefits, and education reimbursement of $5,250 annually
  • Additional compensation for visits over your caseload

This position will travel to Good Samaritan Hospital

Job Details

Our Home Care Transition Nurses:

  • Conduct patient assessments and develop individualized transition plans
  • Coordinate referrals and facilitate smooth transitions to home care services
  • Build and maintain strong relationships with physicians, discharge planners, and referral sources
  • Serve as a key ambassador for Catholic Home Care within the community
  • Identify opportunities for program growth and service expansion
  • Deliver exceptional customer service and support to patients, families, and healthcare partners
  • Collaborate with interdisciplinary teams to ensure positive patient outcomes

Requirements:

  • Current Registered Nurse (RN) license in New York State
  • Minimum of 2 years of home care experience, including patient assessments
  • Strong relationship-building, communication, and customer service skills
  • Experience in healthcare marketing, referral development, or business growth is a plus
  • Passion for patient advocacy and care coordination
Posted Salary Range

USD $90,000.00 - USD $105,000.00 /Yr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
