Position Summary for Admissions Registered Nurse (RN):

An admissions nurse is a registered nurse and an Interdisciplinary Team (IDT) member whose primary responsibility is to present ANX Hospice Care services to patients and families in a manner that results in the admission of appropriate patients. An admissions nurse performs consultation and admission visits to assess the patient's needs and completes admission assessment and contractual agreement documentation for Hospice.

  • Possesses an understanding of the Hospice program and benefits.
  • Knowledge of the principles of the intake, admission, and transition to care process with the ability to further develop skills in these competency areas as a Admissions Registered Nurse (RN)
  • Evaluate patients for hospice appropriateness, e.g. medical records review as requested.
  • Documents assessment, current medications and initiates appropriate care plans.
  • Writing and initiating a plan of care that establishes goals, based on nursing diagnosis and incorporates the patient and the family in the planning process.
  • Responsible for assessing patient's clinical status, planning, teaching, and implementing immediate interventions on the day of admission.
  • Coordinates medications and DME needs for same-day delivery.
  • Coordinator with attending physician, Hospice Medical Director regarding patient admission and follow up needs.
  • Communicates admissions information, issues, and concerns inpatient care meetings to assure smooth transition and continuity of care with other assigned staff, including a detailed report to the assigned Case Manager and assigned Social Worker.
  • Performs routine and unscheduled nursing visits when required and serves as a backup to other field staff when there are no admissions
  • Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient's family has concerning hospice care.
  • Participates in the weekend on call rotation, holidays and After Hours as needed
  • The admission nurse is available to do admissions at the patients home, hospital, board, and care or skilled nursing facility.
  • Responsibilities will include but not limited to performs comprehensive nursing evaluations, demonstrates flexibility in scheduling and readily accepts new admissions and emergency assignments as requested, participates in QAPI program as required, adheres to the highest standards of professional and personal conduct in the performance of his/her duties.

Minimum Qualifications of a Admissions Registered Nurse (RN):

  • Must be able to work on weekends in both San Mateo and San Francisco counties.
  • Current CA Registered Nurse (RN) license
  • Current CPR card for Healthcare Providers
  • 1 year of Registered Nurse (RN) experience in Hospice Care.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Understands the philosophy of Hospice concepts. Experience with the terminally ill patient.
  • Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.
  • Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
  • Self-directed and able to work with minimal supervision.
