Job Summary

Are you an compassionate Registered Nurse who thrives on autonomy and delivering high-quality, patient-centered care?

We are seeking a Weekend Hospice Registered Nurse (RN) to provide skilled nursing care, symptom management, and end-of-life support to terminal patients in home and inpatient hospice settings across the Tri-County area. In this field role, you will work with minimal daily direct supervision, leading and coordinating with an interdisciplinary team to ensure our patients and their families receive comfort, dignity, and exceptional care during life's most critical moments.

Key ResponsibilitiesDirect Patient & Clinical Care (60%)
  • Provide comprehensive skilled nursing care, hands-on physical assessments, medication administration, and symptom/pain management in accordance with the SC Nurse Practice Act.

  • Collaborate with physicians to report condition changes, obtain/receive medical orders, and adjust care plans as needed.

  • Educate patients, families, and caregivers on end-of-life care, self-care techniques, and proper use of medical equipment.

  • Maintain complete, accurate, and timely electronic clinical documentation.

Supervision & Coordination (25%)
  • Supervise Care Team (10%): Oversee care provided by Licensed Practical Nurses (LPNs) and Hospice Aides to ensure treatment goals are met; delegate duties appropriately.

  • Interdisciplinary Coordination (15%): Partner with social workers, chaplains, external healthcare agencies, and families. Participate in on-call coverage as required by department needs.

Quality & Continuing Education (15%)
  • Attend Interdisciplinary Group (IDG) meetings and actively contribute to Quality Assessment Performance Improvement (QAPI) initiatives.

  • Maintain professional licensure and skills through ongoing continuing education.

Qualifications & RequirementsLicensure & Certification
  • Active, unencumbered Registered Nurse (RN) license in the State of South Carolina OR an active Compact/Multi-State RN license (if non-SC resident).

  • Active CPR Certification.

Experience & Skills
  • Strong clinical assessment, symptom management, and critical thinking skills.

  • High degree of autonomy, organizational ability, and independent time management in field settings.

  • Exceptional oral and written documentation skills.

Transportation & Other Prerequisites
  • Must have reliable personal transportation to visit patients in home settings.

  • Valid South Carolina Driver's License with a good driving record.

Physical Demands & Work Environment
  • Field-Based Role: Requires driving throughout assigned zip codes under varying weather and road conditions.

  • Physical Activity: Frequent standing, walking, bending, stooping, crouching, and patient transfers/lifting (intermittent lifting up to 50 lbs, frequent pushing/pulling over 50 lbs).

  • Environment: Direct patient contact in residential and facility environments, with standard health care risks (exposure to body fluids, communicable diseases, and pharmaceuticals).
