Job Title: Registered Nurse (RN) - Home Health Case ManagerService Area
  • Serving patients across Fox Valley and Oshkosh

  • Community-based home health role requiring travel between patient homes


Position Overview

We are seeking a compassionate and organized RN Case Manager to deliver high-quality home health services. This role involves managing a patient caseload, coordinating care, and ensuring optimal outcomes through comprehensive assessments and ongoing patient support.


Key Responsibilities
  • Manage and coordinate a caseload of home health patients

  • Perform initial and ongoing assessments (physical, social, environmental)

  • Develop and update plan of care in collaboration with physicians

  • Provide direct patient care and adjust interventions as needed

  • Complete accurate and timely documentation, including:

    • OASIS

    • 485 forms

    • Verbal orders

  • Communicate regularly with physicians and care team members

  • Facilitate discharge planning and continuity of care

  • Participate in quality improvement initiatives and chart audits

  • Deliver patient and family education

  • Ensure compliance with agency, state, and federal guidelines


Unit & Caseload Information
  • Approximate census: 165 patients

  • Average productivity: ~6 visits per day

  • First-time travelers: Accepted

  • Minimum experience: 1 year RN experience


Patient Population
  • Adult and geriatric patients

  • Common conditions include:

    • Wound care

    • COPD, CHF, Diabetes

    • Post-operative (total joints)

    • Medication management

    • IV therapy and PICC line care


Required Qualifications
  • Active RN License (state-specific)

  • Associate's Degree in Nursing (ADN) or higher

  • Valid Driver's License and reliable transportation

Certifications:
