RN 7/7 On-Call
Position Overview
The 7/7 salaried On-Call Registered Nurse is experienced in nursing and handles a variety of hospice nursing care depending on the current needs of patients and the organization. Duties may include hospice patient visits, admissions, responding to urgent night and weekend patient needs, and/or filling in for other nursing staff. The On-Call RN works from 5:00 PM to 8:00 AM Monday-Thursday, and 24 hours on weekends Friday-Sunday, following a rotating schedule of 7 days on and 7 days off.
Essential Functions and Skills
Patient Care
  • Available to receive and respond to calls from Triage during assigned shifts making visits to patients with changing conditions and completing weekend admissions.
  • Completes comprehensive and ongoing assessment of patient and family to determine hospice needs for after-hours calls. Provides a complete physical assessment and history of current and previous illnesses.
  • Provides professional nursing care by utilizing all elements of the nursing process.
  • Assesses and evaluates patient's status by:
  • Writing and initiating Plan of Care
  • Re-evaluating patient and family/caregiver needs
  • Participating in revising the plan of care as necessary
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a plan of care that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with the activities of daily living and facilitates the patient's efforts toward self- sufficiency and optional comfort care.
  • Responsible for after-hours on-call duties include remaining available for all calls related to patient care while making appropriate visits as necessary.
  • Remains with patient and/or family through a crisis period until resolved and/or relieved.
  • Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management and changes/outcomes as appropriate.
  • Responsible for maintaining compliance with all HIPAA regulations and requirements. Treats all patient information as confidential and utilizes protected health information on a minimum necessary basis.

Requirements/Qualifications
  • Must have a current Registered Nurse (RN) license in state(s) of practice.
  • Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR). Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact.
  • Graduate of an accredited school of nursing required.
  • Ability to pass background, consumer report and drug test.
  • If Driving: Valid State Driver's License, automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.

Preferred
  • 1+ years of recent experience in hospice/home care preferred.
  • 2+ years of Nursing experience, preferred
